Abstract
Multidisciplinary Discharge Planning Rounds can improve patient satisfaction with care by improving communication among the healthcare team. This original work provides a real-world, practical framework for nursing leaders who are interested in promoting teamwork, communication, and collective decision-making to improve patient outcomes.
