Abstract
The Transitional Nursing History (TNH) Tool introduces pre-licensure students to transitional healthcare planning needs and nurses' essential roles. The TNH represents a collaboration between inpatient and community health clinical sites and promotes nursing students' understanding of the importance of transitional healthcare planning for patients/families and the impact on health outcomes.
