Abstract
Shared medical appointments offer an innovative approach to chronic disease management and allow for positive engagement by patients with self-management. Using a multidisciplinary approach to shared medical appointments with congestive heart failure patients shows a reduction in depression, and an increase in quality of life and health promotion behaviors.
