Abstract

Type-2 Diabetes Mellitus is a serious,chronic condition that must be managed properly. Lowering hemoglobin A1C (HbA1c) levels to less than 7% has been shown to reduce vascular complications. Nurse Practitioner (NP) students in collaboration with a licensed NP implemented a change model that positively impacted the health of a population.
