Abstract
Community health nurses responsible for managing the care of sick patients have a responsibility in knowing how to prevent the progression of diseases and the promotion of self-management skills. There is a need to incorporate strategies that empower patients to be integral members of the health care team. This process can be achieved by breakdown communication barriers that assume that the health care provider has given the patient the appropriate health information
