Abstract

Congestive heart failure is a complex and debilitating condition affecting millions of people nationwide resulting in frequent admissions to the hospital and decreasing patient's quality of life and costing thousands of dollars not only to patients by to the healthcare system. The proposed quality improvement project is aimed at reducing Heart Failure 30-day hospital readmission rates by implementing a post-discharge educational program. The program consists of weekly contact with heart failure patients recently discharged from the hospital to reinforce discharge education topics.
