Abstract

This project aims to implement an educational initiative to improve self-efficacy in patients with type 2 diabetes and cardiovascular diseases. The population, intervention, and outcome (PIO) question addressed by this study is the following: Among adults between the ages of 18 and 64 with T2D mellitus (T2DM) and CVD or at risk of CVD, would a patient-centered education model, the Diabetes Cardiometabolic Self-Management Education, and support (DCMSMES), improve the patient's self-efficacy in self-care behavior?
