Abstract
Hypertension is one of the most modifiable risk factors for the prevention of heart disease. Current practice involves medical management of hypertension by the primary care provider. Provision of medical management alone is not improving blood pressures of patients with hypertension. Therefore, a patient-centered interprofessional team was developed to address the lack of a coordinated approach to the management of hypertensive patients.
