Abstract

Project aims: To increase Advance Care Planning (ACP) discussions by 10%, increase identification of end-of-life (EOL) patients by 10% and evaluate the effectiveness of an Acute Care Team (ACT) intervention on EOL patients in the Emergency Department (ED) by October 31st, 2017.
