Abstract
Poor nutrition is the leading cause of chronic disease and disability plaguing the US and many other nations today. Lifestyle medicine uses evidence-based therapeutic interventions and highlights the plant-predominant diet to address the root cause of illness. The care manager and nursing role is vital to empower patients to take control of their health to prevent and in many cases greatly improve chronic illness. This presentation will provide key strategies to promote behavior change.
