Abstract
Introduction: CRNAs are in a unique position to directly impact the surgical experience for veterans with PTSD during and after general anesthesia. The purpose of this descriptive study is to evaluate anesthetic management practices utilized by CRNAs for veterans diagnosed with PTSD undergoing general anesthesia using a survey approach for data collection.
Methods: Survey participants were recruited from a public membership list of the Association of Veteran Affairs Nurse Anesthetists (AVANA) website. The 10-question survey was developed through Survey Monkey to determine methods of identifying patients with PTSD, the likeliness of a change in anesthetic plan based on PTSD, whether or not participants had training in PTSD identification, and explore the concepts of pharmacologic and non-pharmacologic interventions used by CRNAs when caring for Veterans identified with PTSD.
Results: Chart review was the most common (91%) method used to identify Veterans with PTSD. Once PTSD status was recognized, participants were either very likely (51%) or somewhat likely (25%) to follow-up on this knowledge during the pre-operative period. Non-pharmacological interventions that were very likely to be utilized in Veteran patients for the prevention of ED included maintaining a quiet environment upon emergence (77%), building rapport preoperatively by providing perioperative plan of care with patient (83%), discussing previous anesthetic experiences or fears of the unknown regarding operation (77%), verbal anxiety reduction (78%), safety reassurance (84%), and frequent pain reassessments (65%). Pharmacological interventions that were very likely to be used in the prevention of ED in Veterans with PTSD included precedex (59%) and propofol (54%).
