Abstract
Health care reform is a topic at the forefront of discussion between all disciplines of the health care community. Heart failure is commonly discussed within the medical community as a diagnosis that is commonly linked to high health care cost and early hospital readmission rates. The purpose of this program project was to develop an evidence-based guideline to be used in an innovative, paramedic, health care delivery program for patients with heart failure that will direct paramedics working in an expanded role by defining the Emergency Medical Service Prevention program's admission criteria, the assessment activities to be completed at each home visit, and patient outcome measures. This 10 week structured heart failure - evidence based guideline will help patients implement a health-promoting behavior each week for 8 weeks and assess their success in implementing each HPB in weeks 9 and 10 using a health promotion model. At each visit, the paramedic will perform 3 distinct types of structured activities that consist of history review, an identified health promotion behavior education, and finalization of the visit. Visits are uniform in content except the identified health promotion behavior changes weekly for the fist 8 weeks. A health promotion behavior is presented each week using a health promotion model, which consists of 9 concepts further divided into 3 parts for educational purposes. A lesson plan allowing paramedics to validate on his or her use of the evidence-based guideline has been development.
