Abstract

In response to a healthcare provider shortage, the Advanced Practice Registered Nurse (APRN) workforce is rapidly expanding with more practitioners entering clinical practice than ever before. Unfortunately, once APRNs enter the workforce, decreased job satisfaction, role strain, and increased job demands may contribute to high turnover rates. To increase institutional support for novice Advanced Practice Providers (APPs) during the transition period, a large academic quaternary pediatric hospital in the southwestern United States developed a mentorship subcommittee for APPs with fewer than two years of advanced practice experience. This quality improvement project evaluated the impact of formal mentorship on job satisfaction and turnover during the initial transition period using a descriptive longitudinal quality improvement pilot feasibility project design. Outcomes were evaluated and analyzed between cohorts using the Novice Nurse Practitioner Role Transition (NNPRT) scale and human resource data. Similar analyses examined association of demographic, personal characteristics and mentorship with job satisfaction and turnover. Analysis of the results show overall improved NNPRT scores for novice APPs who receive formal mentorship, compared to those who do not. Moreover, data suggests a positive correlation between formal mentorship and intent to stay in role for two years, though a reduced intent to stay for five years. The recommendation is for broader studies across institutions and geographies to improve generalizability.
