Abstract
Purpose: To review women younger than 21 who received cyclophosphamide and the effects on reproductive health
Methodology: A retrospective chart review is in progress (data collection phase). Descriptive statistics will be utilized.
Results: All results are not currently available as this is a study in progress but will be available at the NPWH conference. We felt this study is important as systemic lupus erythematosus (SLE) is an autoimmune disease that primarily affects women of reproductive age. Cyclophosphamide (CYC) can have long term effects on menstruation and fertility. There have been few studies looking at the reproductive health of women younger than 21 who received CYC. Texas Children's Hospital has one of the largest cohorts of SLE patients under 21 who received CYC. We also felt this study is warranted as we are initiating a quality improvement project at our institution to improve care of reproductive health in our SLE patients. CYC is typically used in severe cases of SLE where there is renal or central nervous system involvement. CYC can have several effects on the reproductive health as it is teratogenic and can effect menstruation. CYC can be toxic to the ovaries resulting in premature ovarian insufficiency (POI). The risk for POI is dependent of the patient age at the time of exposure and cumulative dose of CYC. Gonadotropin-Releasing Hormone Analog (GnRH-a), such as depot leuprolide acetate, is sometimes given concurrently with chemotherapy for ovarian protection although it is still considered experimental. Women receiving CYC must also be counseled about contraception and the importance of avoiding pregnancy due possible teratogenic effects associated with this medication.
