Abstract
Purpose: To increase providers' usage of the two 'Assist' and 'Arrange' steps of the 5 A's Model in the areas of weight management action plans, dietitian referrals, and follow-up appointments. The 5 A's Behavioral Change Model, originally developed for smoking cessation, has demonstrated efficacy in weight management and is grounded in behavior modification theory.
Summary: In a military treatment facility in the Southwest US, the overweight and obesity rates of active duty, veterans, retirees, and dependents mirrored the national average. Clinicians within the women's health clinic assessed and advised obese clients using verbal and written information about diet and exercise, but rarely made referrals to the dietitian, developed weight management action plans, or made follow-up appointments as recommended by the U. S. Preventive Services Task Force and American College of Obstetrics and Gynecologists. A one-hour training program for weight management counseling using the 5 A's model was provided to a nurse and pharmacist that have opportunities to discuss weight with clients. A ten week pre- and post-intervention chart review for evidence of dietitian and bariatric referrals, weight management action plans, and follow-up appointments assessed the impact of the training in the 'Assist' and 'Arrange' steps of the 5 A's model.
Outcomes: Percentage of patients receiving weight management counseling increased in the post-intervention group (90%; n = 27) compared to the pre-intervention group (63%; n = 19) (p = 0.02) and dietitian referrals increased by 120 percent following the intervention (n = 11), although not statistically significant (p = 0.08).
It is estimated that obesity among active duty military has more than tripled within the last 15 years. Women associated with the military (active duty, reserve, veteran, and dependents) are a special population that is disproportionally at risk for overweight and obesity. The effect of deployments, stress, environment, disordered eating may contribute to weight gain. Multi-component weight management counseling using the 5 A's model can be used by providers across disciplines to partner with clients to adopt healthy behaviors.
