Abstract

Purpose: To address provider barriers to increase awareness of postpartum depression (PPD) and facilitate screening of PPD within eight weeks following introduction of the Postpartum Depression Screening Toolkit (PPDST).

Summary: A pre-test, post-test instruction program with materials (PPDST) was used. Physicians were invited to attend one of two in-services on use of the PPDST. The PPDST consisted of basic PPD education, the Edinburgh Postpartum Depression Scale (EPDS) a reliable and valid, 10 item questionnaire with an algorithm for follow up, counseling scripts for assessment and treatment, a pharmacological information chart, hospital admission protocols, reimbursement codes, an extensive resource/referral list, and referral form templates. Medical assistants (MAs) were invited to attend an in-service to review their role in helping the physician to facilitate PPD screening.

Outcomes: Pre/Post Knowledge Assessment Test using a one tailed paired samples t-test indicated a significant gain in knowledge (T (12) = - 8.314, P < 0.001, d = -2.306); Physicians scored higher on post-test (M = 93.23, SD = 8.447, n = 13) than on pre-test (M = 72.92, SD = 10.642, n = 13); effect size was large. Pre/Post EPDS Screening Assessment Chart Review using a chi squared test found a significant improvement in PPD screening (Chi-Square = 38.394, df = 1, p < 0.001); There was a 32.6% increase in the number of charts screened with EPDS at eight weeks post intervention. All physicians (n = 13) had an increase in test score post-intervention. One of 13 physicians was screening for PPD with EPDS pre-intervention and 10 of 13 physicians were screening with EPDS eight weeks post-intervention. There was a 900% increase in screening rate at eight weeks post-intervention.

Implications for Women's Health: The PPDST was successful in this small group of private practice physicians and proven to be effective in addressing barriers to facilitate PPD screening with EPDS. The PPDST enables the application of Evidence Based Practice to care of post-partum women. It facilitates inter-professional collaboration to improve patient and population health outcomes and fosters the application of clinical prevention and population health measures to improving the health of community & nation.
