Abstract
The University at Buffalo School of Nursing (SON) initiated a community based interprofessional education activity using Ohio State University"s Million Hearts ® curriculum. The SON guided formation of an academic-community health coalition. Community based Million Hearts® screenings were completed by students from nursing, exercise and nutrition science, medicine and pharmacy.
