Abstract
NP-led transitional care medical house call helps reduce polypharmacy in homebound senior population during transition from higher level of care to home. The NP visit also addresses point-of-care challenges such as delay in PCP visits therefore need for prescriptions, and multiple co-morbidity therefore high risk for readmission.
