Abstract
Adults hospitalized for serious mental illness (SMI) have high rates of hospitalization and re-hospitalization. The purpose of this study is to evaluate the effect of a novel transitional care program (TCP), on psychiatric hospital rates and days hospitalized among a targeted number of commercially insured patients.
