Abstract
One academic university created interprofessional didactic and clinical learning experiences for students within the Schools of Nursing, Social Work, Health Professions-Rehabilitation Counseling, and Pharmacy that resulted in improved models for patient care delivery. The nurse-led FQHC uses the model and has implemented innovative approaches to engage/keep patients in care.
