Abstract
Improving support for bereaved populations in hospitals is a priority in palliative care. Bereavement risks are not routinely assessed in hospitals, creating gaps in policy and practice. Using the clinical microsystems approach to quality improvement, a bereavement risk assessment tool was selected and implemented in a hospital setting.
