Abstract

Early pregnancy loss (EPL), also known as miscarriage or spontaneous abortion (SAB), during the first trimester occurs in as many as 25% of all known pregnancies and can lead to significant emotional and clinical morbidities (Al Wattar et al., 2019). It is diagnosed when a non-viable intrauterine pregnancy is identified at less than 13 weeks gestational age (Shorter et al., 2018). While this diagnosis can be tragic for patients, it can also be very complicated and stressful for providers to manage. Management of EPL involves the consideration of a multitude of factors. Providers must discuss implications and management options with consideration of a patient's health status, symptoms, social support, and age. Options for management include expectant management, medical management, or surgical aspiration with a physician. Follow-up and mental health support are also crucial during care. In a large clinic with multiple providers, management plans are confusing, and follow-up often gets overlooked. After a review of current clinical problems and personal experience with the issue of EPL at the Vanderbilt Midwives clinical practice, a focus on implementation of a standardized EPL management guideline was chosen as a DNP project.
