Abstract
Diabetes is a complex medical condition that, if undertreated, can place individuals at increased risk of multiple comorbidities. Managing diabetes is a collaborative effort of both the individual and the healthcare team. Modifications in medical management may occur as the patient transitions through various clinical settings. Due to these transitions through different clinical settings, healthcare providers play an essential role in maintaining medical management, screening, assessing, and informing patients of community resources. Individuals who are motivated, encouraged, and engaged in managing their healthcare will have optimal healthcare outcomes. This DNP project will address the identified gaps in diabetes management and the utilization of a multimedia tool to improve motivation, encouragement, and engagement in diabetes self-management.
